Job Summary
You will lead the care coordination and social services team, overseeing planning, organizing, and directing all activities, staffing, and performance improvement in clinical services and reporting needs. You will collaborate with various departments and stakeholders to ensure seamless patient transitions and provide care management guidance.
Responsibilities
- Oversee care coordination and social services, including planning, organizing, and directing all activities.
- Manage staffing and performance improvement in clinical services such as reducing length of stay (LOS).
- Collaborate with departments and post-acute service providers to streamline patient transitions.
- Consult and collaborate with managers, physicians, administration, and community healthcare workers on care management issues.
- Tackle tough conversations and educate others on care coordination best practices.
Requirements
- Bachelor's Degree in Nursing (BSN) or Master's in Social Work (MSW) or equivalent education and experience.
- Minimum of 5 years of clinical care coordination experience or 5 years of healthcare management experience.
- Extensive operational experience, program planning, implementation, staff development, and needs assessment.
- Comprehensive knowledge of care coordination and discharge planning.
- Current California RN license or LCSW license, and national certification in case management or case management.
