Director Care Coordination
About the role
The Director of Care Coordination for St. Mary Medical Center is responsible for the overall direction and management of care coordination and social services. This includes planning, organizing, and directing all activities, staffing, performance improvement in the delivery of clinical services, such as LOS reduction, and reporting needs within the Medical Center and Dignity Health.
You will work closely with all departments at the Medical Center and post-acute service providers to streamline patient transitions through the health care system and into the community post-discharge. Additionally, you will consult and collaborate with other managers, physicians, administration, and community-based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.
Responsibilities
- Overall direction and management of care coordination and social services
- Planning, organizing, and directing all activities related to care coordination and social services
- Staffing and performance improvement in the delivery of clinical services
- Collaboration with other departments and post-acute service providers to ensure smooth patient transitions
- Consultation and collaboration with managers, physicians, administration, and community-based healthcare workers on care management issues
Requirements
- Bachelor's Degree in Nursing (BSN) or Master's in Social Work (MSW) or equivalent education and experience required
- Minimum of 5 years of clinical care coordination experience (Utilization Management, Denial Management, Case Management, Care Coordination), or 5 years of progressively responsible healthcare management experience required
- Extensive operational experience, program planning, implementation, staff development, and needs assessment in healthcare environment required
- Comprehensive knowledge of care coordination and discharge planning required
- Current California Registered Nurse (RN) license, or California Licensed Clinical Social Worker (LCSW) required
- National certification in either Certified Case Manager (CCM) or Accredited Case Manager (ACM) required within 2 years upon hire
