Inpatient Unit RN Case Manager
About the Role
The Inpatient Unit (IPU) Registered Nurse Case Manager is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation. The IPU Registered Nurse Case Manager functions as a member of the Interdisciplinary Team (IDT) to provide routine and emergency assessment of patients, education to patients and caregivers, and care coordination to meet the needs of patients and their families. This role helps maintain the patient and family’s maximum level of comfort and coping, works a flexible schedule to meet agency staffing needs, and provides after-hours emergency support (on-call responsibilities are required). The role also ensures compliance with state and federal regulatory agencies.
Responsibilities
- Coordinates clinical, psycho-social, and spiritual services as indicated by the Plan of Care through a case management model.
- Collaborates with the IDT to meet patient needs for clinical, psycho-social, and spiritual interventions.
- Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents.
- Submits appropriate documentation and paperwork to facilities at the completion of patient visits.
- Assists in identifying the need for intervention of other IDT members.
- Effectively communicates patient and family needs to the IDT.
- Performs supervisory duties for Hospice Aides and Licensed Practical/Vocational Nurses and adjusts assignments as needed to promote ideal patient outcomes.
- Communicates with IDT, Medical Director, and Attending Physician regarding measures to alleviate symptoms and monitors response to implemented measures.
- Provides physical and emotional care to support the patient and family in periods of crisis through case management with the IDT.
- Collaborates with the IDT to coordinate hospice care for the patient and family, ensuring appropriateness, continuity, and quality of care.
- Ensures documentation at bedside and maintains updated, comprehensive care plans reflecting current problems, goals, and interventions.
- Maintains clinical knowledge of appropriate pharmacological and non-pharmacological interventions to manage pain and other symptoms using clinical practice guidelines.
- Completes routine and emergency assessments on each patient as indicated; may include participation in on-call services to provide a holistic team-based approach to hospice services.
- Appropriately records observations, interventions, and other pertinent information.
- Prevents unwanted/unwarranted hospitalizations by meeting patient and family needs in the home through anticipatory care.
- Keeps the Medical Director and attending physician informed of patient changes in condition and requests clinically-sound interventions when needs change.
- Performs other duties as assigned.
Requirements
- Active and unencumbered Registered Nurse license in the state(s) of employment.
- Current CPR certification.
- Associate or Bachelor’s degree in Nursing required.
- Minimum of one (1) year of nursing practice or equivalent experience preferred.
- Hospice, oncology, home health, and/or long-term care experience preferred.
State-Specific Requirements
- Louisiana: Two (2) years of full-time experience as a Registered Nurse; or two (2) years of full-time clinical experience as a Licensed Practical Nurse in hospice may substitute for RN experience.
- Mississippi: One (1) year of full-time experience as a Registered Nurse; or three (3) years of full-time continuous employment as a Licensed Practical Nurse immediately preceding RN licensure.
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, and external parties. Strong written and verbal communication skills.
- Ability to understand, read, write, and speak English.

Physical Demands and Work Environment
The demands of this role necessitate regular standing, walking, and manual dexterity, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and conditions typical to the healthcare field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
