About the Role

The Inpatient RN Case Manager collaborates with the clinical team to coordinate patient care and develop safe discharge plans, aiming to improve patient outcomes and ensure efficient healthcare utilization.

Responsibilities

• Conduct assessments to identify patient needs and create tailored discharge plans, especially for high-risk individuals.
• Work closely with healthcare providers to implement discharge strategies.
• Maintain current knowledge of healthcare regulations, trends, and payer requirements related to patient care and discharge planning.

Requirements

• Registered Nurse license (current Maryland license or eligibility).
• ADN required; BSN preferred.
• 3-5 years of relevant inpatient case management or discharge planning experience.
• Knowledge of healthcare theory and practice within a specialized field.
• Case Management certification preferred within 3-5 years of hire.
