Role Overview
The Inpatient RN Case Manager collaborates with the clinical team and medical providers to coordinate patient discharge plans, aiming to enhance patient outcomes and ensure efficient utilization of healthcare services.
Key Responsibilities
- Assessment & Planning: Conducts initial and ongoing assessments to identify patient needs and develops tailored discharge plans, especially for high-risk patients.
- Intervention & Collaboration: Implements discharge plans through close coordination with healthcare teams and providers.
- Continuous Improvement: Keeps updated on healthcare trends, regulations, and payer requirements related to patient care and discharge processes.
Qualifications & Skills
- Professional knowledge equivalent to a Bachelor’s degree; BSN preferred, ADN required.
- 3-5 years of relevant experience, with inpatient case management or discharge planning experience preferred.
- Current RN license in Maryland or eligibility to obtain it.
- Case management certification is a plus.
