About the Role

The Inpatient RN Case Manager collaborates with the clinical team to coordinate patient discharges, aiming to enhance patient outcomes and ensure efficient healthcare utilization. This full-time position involves assessing patient needs, developing and implementing discharge plans—especially for high-risk cases—and staying updated on healthcare regulations and payer requirements.

Responsibilities

• Conduct initial and ongoing patient assessments to determine care and discharge needs
• Develop focused discharge plans and collaborate with healthcare providers to execute them
• Maintain current knowledge of healthcare trends, regulations, and payer policies

Requirements

• Registered Nurse license with current Maryland eligibility
• BSN preferred; ADN required
• 3-5 years of inpatient case management or discharge planning experience
• Knowledge of healthcare theory and practice within a specialized field
• Case management certification preferred within 3-5 years of hire
