Inpatient Case Manager-RN
About the role
The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primarily focusing on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay. Directly supports patients identified in need of comprehensive case management, promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver.

Requirements
- 12 months nursing experience
- Registered Nurse (RN)
- Basic Life Support (BLS) certification within 90 days
- Bachelor of Science in Nursing (BSN) preferred
- Certification in Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) required within one year of eligibility
- One year acute care case management experience preferred
