About the Role
Provides social work services including assessment, care planning, and case coordination to patients and their families. Offers consultation to healthcare staff and initiates social work care plans. Acts as a liaison between the organization and the community to coordinate services, improve client care, and foster community relationships through outreach activities.
Responsibilities
- Conducting psychosocial assessments, developing and implementing social work plans, and addressing emotional, mental, and behavioral health issues to promote personal and social functioning.
- Managing cases, coordinating services, providing resource referrals, advocacy, and education to individuals, groups, couples, and families, ensuring appropriate placement and support.
- Supporting program development, policy implementation, and quality improvement initiatives, while maintaining clinical documentation and participating in departmental activities and staff training.
- Engaging in research, data analysis, and evaluation of social work practices, ensuring compliance with regulations and standards.
Requirements
- Master’s degree in social work.
- Licensed Social Worker or Licensed Clinical Social Worker in Hawaii.
- Basic Life Support certification from the American Heart Association.
- Experience working with medical diagnoses and community agencies preferred.
- Minimum one year of social work experience preferred.
