On-Call Social Worker

Job Summary Provides comprehensive social work services, including assessment, care planning, case coordination, and consultation to patients, families, and healthcare staff. Acts as a liaison between the organization and the community to facilitate service integration, coordinate care, and strengthen relationships through public relations activities. Responsibilities Conducts psychosocial assessments, develops and implements social work care plans, and provides therapy for individuals, couples, families, and groups. Focuses on preventing and addressing emotional, mental, and behavioral issues to improve personal and social functioning.Manages cases by coordinating resources, offering referrals, advocacy, mediation, and education, ensuring patients are placed in suitable settings.Participates in program development, policy implementation, social work consultation, and quality improvement initiatives. Performs clinical record audits and maintains competency documentation.Engages in research, documents findings per standards and regulations, and ensures proper recordkeeping.Contributes to departmental activities such as meetings, training, and onboarding of new staff. Qualifications Master’s degree in social work.Licensed Social Worker or Licensed Clinical Social Worker in Hawaii.Basic Life Support certification.Experience working with medical diagnoses and community agencies preferred.Minimum one year of social work experience is preferred.
