On-Call Social Worker

About the Role

Provides comprehensive social work services including assessment, care planning, case management, and coordination for patients and families. Acts as a liaison between healthcare providers and community resources to improve patient care and community relationships. Offers consultation to staff and participates in program development, policy implementation, and quality improvement initiatives.

Responsibilities

• Conducts psychosocial assessments, develops and evaluates social work care plans, and provides therapy for individuals, couples, families, and groups. Focuses on preventing psychosocial dysfunction and promoting social and emotional well-being.
• Performs case management, resource coordination, referral services, advocacy, and education. Ensures appropriate patient placement and service delivery.
• Assists with program development, implementation, and evaluation. Participates in research, documentation, departmental activities, and staff training.
• Maintains compliance with regulations, conducts record audits, and ensures accurate documentation of services.
• Engages in continuous professional development and quality improvement efforts.

Requirements

• Master’s degree in social work.
• Valid Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license in Hawaii.
• Basic Life Support (BLS) certification from the American Heart Association.
• Experience working with healthcare diagnoses and community agencies preferred.
• At least one year of social work experience is recommended.
