Inpatient/Social Services - On-Call

Job Summary This role involves providing comprehensive social work services, including assessment, care planning, and case coordination for patients and families. It includes consulting with healthcare staff, initiating social work care plans, and acting as a liaison with community resources to enhance patient care and community relationships. Essential Responsibilities Assessments & Care Plans: Conduct psychosocial evaluations, develop and implement social work plans, and provide therapy to individuals, families, or groups. Focus on preventing psychosocial issues and improving social and emotional functioning.Case Management: Coordinate services, manage cases, provide resource referrals, advocacy, and education, ensuring appropriate patient placements.Program Development & Evaluation: Assist in developing, implementing, and assessing social work programs and policies, participate in quality improvement, and maintain clinical documentation.Research & Documentation: Conduct social work research, document findings per standards and regulations, and ensure accurate medical records.Team Participation: Engage in departmental activities, staff meetings, in-service training, and support orientation of new team members. Qualifications & Skills Master’s degree in social workLicensed Social Worker or Licensed Clinical Social Worker in HawaiiBasic Life Support certificationExperience working with medical diagnoses and community agencies preferredStrong networking, assessment, and communication skills
