About the Role

Provides social work services including assessment, care planning, and case coordination to patients and their families. Offers consultation to healthcare staff and initiates social work care plans. Acts as a liaison between the organization and the community to coordinate services, improve client care, and strengthen relationships through public relations activities.

Responsibilities

  • Conducts psychosocial assessments, develops and evaluates social work plans, and provides therapy for individuals, families, and groups to address emotional, mental, and behavioral issues.
  • Applies theoretical knowledge and social values to facilitate change and improve social functioning.
  • Performs case management, resource coordination, referral, advocacy, and education services, ensuring appropriate placement and support for patients.
  • Contributes to program development, implementation, and evaluation, including social work consultation, quality improvement, and departmental activities.
  • Engages in research, documentation, and compliance with regulatory standards, maintaining accurate clinical records.
  • Participates in staff meetings, training, and orientation activities to support departmental functions.

Requirements

  • Master’s degree in social work.
  • Licensure as a Licensed Social Worker or Licensed Clinical Social Worker in Hawaii.
  • Basic Life Support certification from the American Heart Association.
  • Experience working with medical diagnoses and community agencies preferred.
  • Minimum one year of social work experience is preferred.
