Inpatient/Social Services - On-Call

Role: Provides psychosocial assessments and social medicine services as part of the emergency department/CDU care team, addressing patients' social, emotional, and legal needs. Responsibilities Participates in case finding, referral, and delivery of social support for high-need populations.Completes psychosocial assessments, develops and implements care plans, and documents interactions per standards and regulations.Assists patients and support systems with coping strategies, abuse cases, advance care planning, and legal decisions.Provides community resource referrals, supports discharge planning, and coordinates post-discharge placements.Contributes to quality improvement, departmental activities, and staff education. Qualifications Masters in Social Work and an active Licensed Social Worker or Licensed Clinical Social Worker license.Minimum of one year healthcare experience.Knowledge of medical terminology, care processes, community resources, and social impact of illnesses.Strong interdisciplinary team skills, customer service focus, and ability to collaborate with various healthcare professionals.Additional skills include documentation, patient education, and familiarity with community and family dynamics.
