About the role
The Registered Nurse- Inpatient Case Manager is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay.
Responsibilities
- Directly support hospital patients identified in need of comprehensive case management.
- Promote effective utilization and monitoring of health services, collaborate, and communicate with the healthcare team and patient/caregiver to manage care and transitions.
- Develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver.
Requirements
- 12 months Nursing experience required; critical care experience preferred.
- BLS within 90 days of hire.
- Associates of nursing Required; BSN is preferred.
- Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
- 1-year acute care case management experience preferred.
- Experience with community resources preferred.
