In-Home Healthcare RN - Afternoon/Evening Shifts

About the role

We are seeking a dedicated Registered Nurse (RN) to provide skilled nursing care in a home setting, primarily serving adult patients. The role involves delivering and documenting patient care according to physician orders, assessing changes in patient condition, and updating care plans accordingly.

Responsibilities

• Providing quality patient care while adhering to confidentiality and professional standards
• Monitoring patient health, recognizing changes, and reporting to supervisors or physicians
• Maintaining accurate documentation of assessments, interventions, and responses
• Educating patients and families on disease management, self-care, and prevention
• Implementing infection control measures and complying with all regulations and policies
• Participating in ongoing education and skill development

Qualifications

• Active, unencumbered RN license in the practicing state
• Current CPR certification and TB test within the past 12 months
• Ability to work in diverse environments, respond to emergency situations, and travel as needed

Skills

• Preferred experience in clinical assessments, documentation, and home health care
• Reliable transportation and physical ability to perform patient repositioning and lifting up to 50 pounds
• Strong organizational, communication, and problem-solving skills
