In-Home Healthcare RN - Night Shifts

About the role

Seeking a Registered Nurse (RN) to provide skilled home healthcare in San Antonio, primarily for adult patients with complex needs such as feeding tubes, tracheostomies, ventilators, and medication administration. The role involves delivering and documenting high-quality patient care, assessing patient conditions, updating care plans, and implementing appropriate nursing interventions to achieve positive outcomes.

Responsibilities

• Coordinate patient care in accordance with physician orders and care plans
• Monitor and report changes in patient status
• Document assessments, interventions, and responses accurately and timely
• Provide patient and family education on disease management and self-care
• Maintain infection control standards and comply with regulations
• Participate in ongoing education and skill development

Qualifications

• Active, unencumbered RN license in the practicing state
• Current CPR certification
• TB test within the past 12 months
• Reliable transportation
• Preferred: six months of clinical experience; home health experience is a plus
