In-Home Healthcare RN - Afternoon/Evening Shifts

We are seeking a dedicated Registered Nurse (RN) to provide skilled nursing care in a home setting, ensuring high-quality patient outcomes. The role involves Delivering and coordinating patient care according to physician ordersAssessing patient conditions continuously and reporting changes promptlyDocumenting assessments, interventions, and responses accurately and timelyDeveloping, implementing, and updating individualized care plansRecognizing and responding to changes in patient needs to prevent or reduce risksEducating patients and families on disease management, self-care, and preventionMaintaining infection control standards and adhering to regulatory policiesParticipating in ongoing education to maintain clinical competencies Qualifications Include An active, unencumbered RN license in the practicing stateCurrent CPR certificationTB test within the last 12 monthsReliable transportationPreferred: Six months of recent clinical nursing experience; home health experience is a plus Skills and abilities: attention to detail, time management, effective communication, problem-solving, and the ability to work in diverse environments. Physical requirements include mobility, strength, and the ability to respond to emergencies.
