Responsibilities
- Providing and documenting skilled nursing care per care plans and physician orders
- Assessing patient conditions, reporting changes, and implementing appropriate interventions
- Developing, updating, and executing nursing care plans
- Educating patients and families on disease management, self-care, and prevention
- Maintaining compliance with policies, regulations, and safety standards
- Participating in ongoing education and skill development
- Providing patient-centered care for adult patients with complex needs, including ventilator, tracheostomy, and feeding tube management
- Monitoring patient status, recognizing changes, and coordinating care with the healthcare team
- Ensuring infection control and safety protocols are followed
- Documenting all nursing activities accurately and timely
Qualifications and Skills
- Active, unencumbered RN license in the practicing state
- Current CPR certification
- Experience in clinical assessments, documentation, and compliance
- Reliable transportation and physical ability to perform patient care tasks
- Strong attention to detail, organization, and communication skills
- Ability to work in diverse environments and respond effectively to emergencies
