About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting, primarily on weekends.
Responsibilities
- Deliver and document patient care per physician orders
- Assess patient conditions and update care plans accordingly
- Educate patients and families
- Adhere to infection control standards
- Ensure compliance with all regulations and policies
Qualifications
- Active, unencumbered LPN/LVN license
- Current CPR certification
- TB test within the last 12 months
- Reliable transportation
- Prior clinical experience, especially in pediatrics or home health, preferred
