About the role
Join a compassionate healthcare team as a Licensed Vocational Nurse (LVN) providing skilled nursing care in home settings.
Responsibilities
- Delivering and documenting patient care per physician orders and care plans
- Assessing patient condition, recognizing changes, and reporting appropriately
- Educating patients and families on disease management and self-care
- Ensuring infection control and maintaining confidentiality and professional standards
- Participating in ongoing education and skill development
- Providing quality patient care, documenting activities, collaborating with the healthcare team, and ensuring safety standards are met in a variety of home environments
Qualifications
- Active, unencumbered LVN license in the practice state
- Current CPR certification and TB test
- Preferably, six months of clinical nursing experience; home health experience is a plus
- Reliable transportation and physical ability to lift 50 pounds and reposition patients
