In Home Healthcare LVN - Part Time Weekend Days

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, primarily for adolescent patients with acuity involving feeding tubes and medication administration (training available). Key Responsibilities Include Delivering and documenting patient care according to physician orders and care plans.Monitoring patient conditions, assessing needs, and reporting changes promptly.Implementing nursing actions to achieve desired outcomes while maintaining confidentiality and professional standards.Providing patient and family education on disease management, self-care, and prevention.Ensuring infection control and compliance with regulations and policies.Participating in ongoing education and skill development. Qualifications And Skills Active, unencumbered LPN/LVN license in the practicing state.Current CPR certification; TB test within 12 months.Preferred: Six months of clinical nursing experience; home health experience is a plus.Reliable transportation, strong attention to detail, organization, and communication skills. Physical and environment requirements include: ability to lift 50 pounds, travel, work in diverse environments, and respond effectively to emergencies.
