In Home Healthcare LVN - Part Time Days

About the role

Join a compassionate healthcare team as a Licensed Vocational Nurse (LVN) providing skilled nursing care in a home setting. This role emphasizes compassionate patient care, documentation, safety, and professional growth, with flexible scheduling options and support from a dedicated team.

Responsibilities

• Deliver and document patient care per physician orders and care plans.
• Assess patient conditions, report changes, and implement appropriate nursing actions.
• Provide patient and family education on disease management and self‑care.
• Maintain compliance with policies, regulations, and safety standards.
• Participate in ongoing education and skill development.

Qualifications

• Active, unencumbered LVN/LPN license in the practice state.
• Current CPR certification.
• TB test within the last 12 months.
• Preferred: six months of clinical experience, home health background.
• Reliable transportation required.

Physical & Environmental Demands
