ICU Clinical Pharmacist-Pharmacy

Responsibilities

  • Works in a multidisciplinary environment alongside medical residents, attending physicians, nurses and auxiliary medical staff in the Intensive Care Units.
  • Provides pharmacotherapy services, executes medication profile review, responds to drug information questions, completes order verification, and provides didactic teaching of both pharmacy and medical learners.
  • Solves patient- and drug-related problems and makes decisions regarding drug prescribing, monitoring, and drug regimen adjustments.
  • Shares responsibility for patient care outcomes by addressing any needs that might arise and participating on various hospital committees.
  • Willingness to precept advanced practice pharmacy students and PGY1 Pharmacy Practice residents (if facility is a PGY1 practice site).
  • Performs other duties as assigned.

Requirements

  • Minimum Education: B.S. Pharmacy or PharmD from an ACPE accredited School of Pharmacy.
  • Minimum Work Experience: PGY2 in Emergency Medicine or Critical Care OR 3 years of pharmacist experience in Emergency Medicine or Critical Care.
  • Required Licenses: [Ohio, United States] Pharmacist Licensed as Pharmacist in the State of Ohio, BLS and ALS.
  • Skills: Strong therapeutic knowledge of a variety of both acute and chronic disease states, strong patient interviewing skills, strong knowledge of pharmacokinetics for vancomycin and aminoglycoside dose initiation and management, strong oral, written, and presentational skills, ability to work in emergency and/or crisis situations, ability to develop and deliver effective interprofessional education, ability to develop strong interpersonal and interprofessional relationships, flexible attitude and ability to adapt to schedule and workload changes, professional presentation and demeanor, collaboration skills to co-manage patient care with various other health care disciplines.

Skills

  • Strong therapeutic knowledge of a variety of both acute and chronic disease states including but not limited to: Cardiovascular disorders, post-cardiac surgery, glucose management, hyperglycemia emergencies, cirrhosis, pulmonary disorders, GI bleeding, hepatorenal syndrome, HIT, coagulopathy, Intraabdominal infections, pneumonia, sepsis, intracranial hemorrhage, nutrition support (TPN/PPN) and fluids, refeeding syndrome, oncologic emergencies, acid-base abnormalities, acute renal injury, electrolyte abnormalities, pain/agitation/delirium, trauma, TTM, stroke, status epilepticus, subarachnoid hemorrhage, TBI, infectious diseases, alcohol withdrawal, ARDS, shock, and medication safety.
  • Strong knowledge of pharmacokinetics for vancomycin and aminoglycoside dose initiation and management.
  • Strong patient interviewing skills for transitions of care coordination and medication reconciliation.
  • Ability to conduct research and perform literature evaluation.
  • Precepting skills for pharmacy students and residents.
  • Ability to develop and deliver effective interprofessional education.
  • Ability to develop strong interpersonal and interprofessional relationships.
  • Flexible attitude and ability to adapt to schedule and workload changes.
  • Professional presentation and demeanor.
  • Collaboration skills to co-manage patient care with various other health care disciplines.
