Clinical Pharmacist Specialist - ICU

Role Overview

Seeking a Clinical Pharmacist Specialist to enhance patient outcomes in the ICU by collaborating with a multidisciplinary team.

Key Duties

• Participate in daily ICU rounds
• Provide clinical pharmacy services including order verification, pharmacokinetic analysis, warfarin and TPN consultations
• Support research and committee initiatives
• Mentor pharmacy residents and students
• Evaluate medication therapy and monitor patient progress
• Contribute to performance improvement and drug cost reduction programs
• Assess patient health status and medication profiles for optimal outcomes
• Participate in professional development, research, and quality assurance activities
• Lead educational efforts for pharmacy staff, residents, and students
• Implement drug cost reduction initiatives
• Engage in clinical decision-making, therapy monitoring, and documentation

Qualifications & Skills

• Doctorate in Pharmacy (PharmD)
• Minimum 2 years of experience; 2-5 years preferred
• Licensed Pharmacist (RPh) and Medication Therapy Services certification
• PGY1 residency required
• Strong collaboration, clinical, and mentorship skills
