Human Services Case Manager (Part-time)

About the role

This part-time position will match the needs of Aging and People with Disabilities in the community with current federal, state, and local programs or other resources that are available to meet those needs. This is done through case management and by screening applicants for long-term community-based care and nursing homes. The role includes assessing eligibility, redetermining needs, adjusting benefits and services, and acting as an advocate for clients within the community.

Responsibilities

• Conduct case management for aging and disabled individuals
• Screen applicants for long-term community-based care and nursing homes
• Assess eligibility and redetermine needs for services and benefits
• Adjust benefits and services according to client needs
• Act as an advocate for clients and aging and disabled persons in the community
• Interpret and implement policy and procedures
• Prepare and maintain written narrative documentation of actions taken
• Interview clients to obtain information and evaluate next steps
• Visit clients, hospitals, nursing homes, APD offices, and other providers
• Travel to individuals’ homes, hospitals, nursing homes, and other local offices

Requirements

• Bachelor’s degree in Behavioral Science, Social Science, or a closely related field; OR
• Bachelor’s degree in any field and one year of human services-related experience; OR
• Associate’s degree in Behavioral Science, Social Science, or a closely related field and two years of human services-related experience; OR
• Three years of human services-related experience (e.g., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)
• Valid driver’s license and acceptable driving record
• Ability to travel to remote locations in various terrain and weather conditions
• Ability to perform physical tasks such as climbing stairs, walking on uneven terrain, and transporting a laptop

Skills

• Experience managing services for individuals
• Experience interpreting and implementing policy and procedures
• Experience preparing and maintaining written narrative documentation
• Experience interviewing to obtain information and evaluate next steps

Working Conditions

• Part-time position, 20 hours per week (schedule determined at interview)
• Office hours: 8 AM–5 PM, Monday through Friday
• General office environment with frequent travel to client homes, hospitals, nursing homes, and other provider settings
• Exposure to environmental hazards, unsanitary conditions, and contagious diseases (e.g., staph infections, lice, MRSA, COVID-19)
• Potential contact with irrational, hostile, or angry individuals who may become verbally or physically assaultive, or armed
• Potential contact with animals, some of which may be dangerous

Background Checks

• Criminal history and background check required for finalists
• Adverse findings may lead to disqualification
• Must complete U.S. Department of Homeland Security’s I-9 form within three days of hire

Pay

Salary range: $5,193 – $6,904 per month
