About the role
The Human Services Case Manager will provide case management for the Trenton Citadel social service programs under the direction of the Corps Officer, with professional supervision provided by Divisional Headquarters Human Services Staff. The Case Manager will identify the needs of clients and provide outreach within the community.
Responsibilities
- Perform intakes, interviews, and assessments of clients’ needs and follow-up case management with households for other supportive programs beyond emergency assistance.
- Provide comprehensive case management, including assessment of client needs, developing a strengths-based action plan, and identifying internal and external programs to support self-sufficiency.
- Coordinate services with partner agencies to ensure a holistic approach for all clients.
- Support the Pathway of Hope Initiative by enrolling eligible families with children under 18 living in generational/chronic poverty and providing case management in line with Pathway of Hope policies.
- Ensure accuracy and timely submission of records and reports in applicable databases for all community programs and funding sources.
- Maintain accurate, confidential, and updated case records in accordance with The Salvation Army’s record-keeping policies.
- Place food orders, submit reports, organize and manage inventory, and handle paperwork from the local Food Bank.
- Monitor and maintain the general condition and upkeep of the Social Service lobby and reception area.
- Manage the main switchboard, including making calls, taking messages, and addressing voicemails to ensure timely client assistance.
- Assist Corps Officers in overseeing volunteers and community service workers.
- Prepare and maintain Salvation Army statistical reports for timely submission.
- Attend and participate in regular staff meetings, trainings, and individual supervision meetings.
- Uphold the standards and principles of The Salvation Army’s mission and ministry.
- Perform other relevant duties as assigned by supervisors.
Requirements
- Associate’s Degree in Social Work, Psychology, Counseling, or a Human Services-related field from an accredited institution (required).
- Master’s Degree in Social Work (preferred).
- At least 1–2 years of experience in case management or counseling.
- Bilingual (English/Spanish) required.
- Knowledge of Microsoft Office programs (Word, Excel, Access).
- Knowledgeable of Mercer County and community resources.
- Strong interpersonal skills and ability to work as part of a team.
- Excellent written and verbal communication skills, with the ability to establish rapport.
- Ability to maintain confidentiality of client and employee interactions.
- Awareness and sensitivity to cultural and socioeconomic characteristics of populations served.
- Good organizational skills and timely follow-up to ensure concerns are addressed promptly.
- Understanding and appreciation of The Salvation Army’s mission.
