Human Services Case Manager

Oregon Department of Human Services is seeking a Human Services Case Manager to assess and support clients in accessing federal, state, and local programs. This role focuses on improving services for the elderly, people with disabilities, and families in assistance programs.

About the Role

As a Case Manager, you will assess clients' medical, social, and financial eligibility, match them with appropriate services, and monitor their ongoing needs. You will collaborate with healthcare providers, social service agencies, and families to develop and implement care plans while maintaining accurate records.

Responsibilities

  • Screen clients for medical, social, and financial eligibility and assess their medical, physical, and mental condition.
  • Interview clients, physicians, hospital discharge staff, family members, and others to determine service needs.
  • Match clients with providers by determining appropriate service placement and payment level.
  • Monitor ongoing needs and provider services through regular contact, including staffing client needs and training providers.
  • Maintain appropriate records and utilize judgment to recognize health and functional status, including potential abuse and neglect.
  • Communicate program requirements clearly to the public and individuals with mental or physical impairments.
  • Make home visits across Douglas County, using a laptop to navigate mainframe systems and search for eligibility rules.

Requirements

  • A bachelor's degree in Behavioral Science, Social Science, or a closely related field.
  • OR a bachelor's degree in any field and one year of human services-related experience.
  • OR an associate degree in Behavioral Science, Social Science, or a closely related field and two years of human services-related experience.
  • OR three years of human services-related experience (e.g., assisting individuals with economic disadvantages, disabilities, abuse/neglect, substance abuse, aging, or inadequate housing).
  • A valid Oregon driver's license with an acceptable driving record.
  • Ability to work in conditions that may include unclean environments, communicable diseases, second-hand smoke, and pets.
  • Strong computer skills to navigate mainframe systems and search for program regulations.

Essential Attributes

  • Experience assessing client eligibility, documenting case information, and identifying health or well-being risks.
  • Ability to collaborate with healthcare providers, agencies, and families to develop care plans.
  • Compassionate, client-centered support with clear communication and advocacy skills.
  • Initiative in managing caseloads and coordinating services with minimal supervision.
  • Sound judgment to assess complex situations and develop tailored care solutions.
  • Ability to prioritize vulnerable clients and communicate effectively in stressful or crisis situations.

Working Conditions

  • Full-time, on-site position (remote work not available).
  • Schedule: Monday–Friday, 8:00 AM–5:00 PM.
  • Regular home visits in Douglas County, with potential exposure to challenging environments.
  • Frequent contact with emotionally fragile, demanding, or potentially hostile clients/family members.
  • Adherence to multiple federal and state regulations with timely and accurate processing of assignments.

Background Checks

  • Criminal history and background check required for finalists; adverse results may disqualify candidates.
  • No visa sponsorship offered.
  • Must complete the US Department of Homeland Security’s I-9 form within three days of hire (E-Verify used).

Pay

$5,554 – $7,386 per month.

This is a permanent, full-time position represented by the Service Employees International Union (SEIU). This recruitment may be used to fill future vacancies in the same classification.
