Human Services Case Manager
About the role
The Oregon Department of Human Services is seeking a Human Services Case Manager to facilitate Medicaid eligibility and coordinate essential services for individuals requiring long-term care support.
Responsibilities
- Determine initial and ongoing Medicaid eligibility for long-term services and supports (LTSS).
- Develop and implement service plans.
- Coordinate the provision of services for community-based care and nursing homes.
- Regularly review and update Medicaid eligibility and monitor service plans.
Requirements
- A bachelor's degree in a Behavioral Science, Social Science, or a closely related field, OR a bachelor's degree in any field AND one year of human services related experience.
- An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience.
- Three years of human services related experience.
Essential Attributes
- Excellent communication skills to assess client needs.
- Excellent people/customer service skills.
- Experience obtaining information through interviews.
- Experience preparing and maintaining written documentation of actions taken/needed.

Pay
$5,193 - $6,904 per month
Schedule
Monday through Friday, 8am-5pm
Qualifications
Must have a valid driver’s license and acceptable driving record.
Skills
Experience using excellent communication skills, providing excellent people/customer service skills, obtaining information through interviews, and preparing and maintaining written documentation of actions taken/needed.
