Position Summary
The Housing Case Manager provides comprehensive, people-centered support to individuals in recovery from substance use and/or mental health challenges. This position combines traditional case management with direct, hands-on guidance in the community.
Key Responsibilities
- Provide case management services for clients engaged in substance use treatment programs, including assessment, care planning, and service coordination.
- Develop individualized service plans based on client needs, treatment goals, and available resources.
- Cookordinate with clinical staff, peer support specialists, and external providers to ensure continuity of care.
- Connect clients with community resources such as housing, employment assistance, mental health services, medical care, and legal support.
- Monitor client progress, track outcomes, and revise service plans as needed.
- Maintain timely, accurate, and confidential documentation in compliance with agency policies and state/federal regulations.
- Advocate for clients' needs while promoting self-sufficiency and empowerment.
- Participate in interdisciplinary team meetings, case reviews, and supervision.
- Stay current on local resources, behavioral health best practices, and regulatory requirements.
- Accompany clients in the community to practice skills and access resources (e.g., grocery stores, banks, employment centers, recovery meetings).
- Aid clients with life-skills development including: Budgeting and financial management Grocery shopping and meal planning Time management and organization Job readiness and employment support Communication and interpersonal skills
Qualifications
- A high school diploma or equivalent is required.
- An associate’s or bachelor’s degree in behavioral health, Social Work, Psychology, or a related field is preferred.
- A Chemical Dependency Counselor Assistant (CDCA) certification is required.
- A Qualified Mental Health Specialist (QMHS III) certification is preferred.
- Minimum 2-3 years of experience in behavioral health, substance use treatment, or crisis response is required.
- Strong crisis management and de-escalation skills are necessary.
- Familiarity with community resources in the Cleveland, Ohio area is important.
- The ability to maintain professional boundaries and handle sensitive situations with empathy is crucial.
- Maintaining requested billing hours is expected.
- A valid driver’s license and reliable transportation are required.
