About the role
FACETS is seeking a mission-driven Housing Case Manager to join the Next Steps Family Program. The program serves families experiencing homelessness or at-risk of homelessness in region 1 of Fairfax County. The role involves assessing housing barriers, developing case management plans, providing mediation and advocacy, and maintaining consistent communication channels.
Responsibilities
- Assess housing barriers and determine housing and service needs.
- Create and maintain case management plans, including intake interviews and goal attainment.
- Implement the Housing First model to provide housing stabilization services and prevent homelessness.
- Mediate and advocate with landlords on behalf of clients to secure and maintain housing.
- Create and maintain communication channels with various stakeholders.
- Determine appropriateness of households for rapid rehousing funds and track their use.
- Advocate for and assist clients in accessing services such as mental health, intellectual disability, alcohol and drug treatment, housing referrals, financial assistance, home-based services, training, medical services, mentoring, and socialization.
- Maintain a flexible schedule, including evenings and weekends.
- Use automated technology to maintain and report on case data.
- Participate in staff meetings and collaborate with colleagues to achieve FACETS goals.
- Represent FACETS in the community according to the agency's mission, vision, and code of ethics.
- Perform other duties as assigned.
Requirements
- Bachelor’s degree required, preferably in a human service-related field.
- Experience working with homeless and/or at-risk populations, or in emergency shelter settings, or similar case management experience.
- Knowledge of and belief in the Housing First and Rapid Re-Housing philosophies and strategies.
- Ability to work independently and manage a caseload with minimal supervision.
- Excellent data entry and data quality skills.
- Clear and concise communication skills, both oral and in writing.
- Proficiency with Microsoft Office Suite or related software.
- Bilingual Spanish/English proficiency.
- Able to work a flexible schedule.
- Able to lift up to 15 pounds and attend meetings at other FACETS office locations.
Qualifications
- MSW preferred.

Pay
$47,000.00 - $50,000.00 per year.
Schedule
