Essential Duties And Responsibilities

• Provides intake, psychosocial, and substance abuse assessments to and develops an appropriate treatment plan for each client in the assigned caseload.
• Serves as a case manager to assist in the creation and adherence to individual Service Plans, identifying short-term and long-term goals and objectives.
• Conducts home visits, coordinates medical, psychiatric, and substance abuse services, monitors medication compliance as necessary, and maintains written information or case notes.
• Conducts individual and group supportive counseling sessions, organizes and educates tenants through groups and other activities, and links clients to community services.
• Participates in weekly clinical meetings to discuss high-risk clients and updates spreadsheets related to clients' progress.
• Performs other duties as assigned by supervisory staff.

Qualifications

• Bachelor’s Degree in Social Work or related field of experience required.
• One to two years of experience working with homeless mentally ill adults or other special needs populations preferred.
• Excellent oral, writing, and listening skills.
• Proficiency in computer required.
• Bilingual or multilingual a plus.

Pay

$38,000 - $46,000 per year.
