Case Manager

PRIMARY DUTIES

• Oversees and guides the rehabilitation and transition plans for Program Participants.
• Conducts individual and group counseling sessions.
• Prepares and updates case file notes and reports on program clients’ progress
• Conducts program intake screening interviews, and recommends program placement or referrals
• Moderates discussions and workshops on anger management, conflict resolution, and life planning skills.

SECONDARY DUTIES

• Complete various special projects as directed.
• Supervises New Client detoxification process
• Produce various reports for staff, management and board of directors.

JOB REQUIREMENTS

• Valid Calif. Driver’s License & Social Security Card.
• Certification in substance abuse counseling
• Thorough knowledge of 12 Steps
• Able to take initiative and work independently.
• Able to work well with clients, staff, volunteers, visitors and outside organizations.
• Exceptional oral and written communications skills.
• Exceptional organizational skills.
• Strong administrative skills.
• Must understand and embrace the vision of the Midnight Mission.
