Hamilton Health Center is seeking a Housing Case Manager to support individuals experiencing opioid use disorder by promoting housing stability, recovery, and long-term well-being. Key Responsibilities
- Provide case management, care coordination, and advocacy to support client recovery and housing stability
- Develop and implement individualized service and housing plans with clients
- Connect clients to housing resources, employment support, counseling, and community services
- Maintain relationships with landlords and community partners to secure housing opportunities
- Monitor client progress and adjust service plans as needed
- Document all services and interactions accurately and in a timely manner
- Support clients in overcoming barriers to care, including transportation needs
- Collaborate with healthcare providers, agencies, and community organizations
- Participate in required meetings, trainings, and outreach activities
- Ensure compliance with confidentiality, program guidelines, and regulatory requirements
Qualifications
- Preferred: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
- Alternative Qualification Path: Minimum of 10 years of relevant experience
- Willingness to enroll in a related Bachelor’s degree program within 6 months of hire
- Experience: At least 1 year of case management experience preferred
- Experience working with substance use disorder, housing services, or vulnerable populations is a plus
Work Environment
- Community-based role with travel throughout the service area
- Direct client interaction in various settings (clinic, community, housing sites)
- May involve supporting clients with complex social and behavioral health needs
