Responsibilities

  • Conduct comprehensive evaluations of patients' physical conditions, mobility, strength, balance, and functional abilities in their home environment
  • Develop individualized treatment plans with measurable goals focused on improving mobility, reducing pain, and restoring functional independence
  • Implement evidence-based physical therapy techniques including therapeutic exercises, manual therapy, gait training, and balance activities
  • Provide expert guidance on home exercise programs, fall prevention strategies, proper use of assistive devices, and safe mobility techniques
  • Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments
  • Collaborate effectively with physicians, nurses, other therapists, and healthcare team members to ensure comprehensive patient care
  • Regularly reassess patient progress, adjust treatment plans as needed, and ensure adherence to clinical standards and regulatory requirements
  • Evaluate home environments for fall risks and recommend modifications to enhance patient safety

Requirements

  • Graduate of an accredited Physical Therapy program (DPT or equivalent)
  • Valid Illinois Physical Therapist license
  • Current CPR certification
  • Valid driver's license and auto insurance
  • Open to new graduates and experienced physical therapists - comprehensive training provided
  • Willingness to learn WellSky Kinnser EMR system (training included)
  • Strong clinical assessment and critical thinking abilities
  • Outstanding written and verbal communication skills
  • Patient-centered care philosophy with cultural sensitivity
  • Strong organizational and time management capabilities
  • Passion for community service and home-based healthcare

Qualifications for International Candidates
