About the role
Seeking a Physical Therapist (PT) to join a home health care team, facilitating patient recovery in their homes.
Responsibilities
- Conduct initial and ongoing assessments of mobility, strength, and function.
- Administer and interpret tests to set functional goals and outcomes.
- Develop and document personalized plans of care, adjusting as needed.
- Provide therapy services per physician orders and ensure proper discharge planning.
- Collaborate with healthcare teams, educate patients and caregivers, and supervise staff such as PTAs and HHAs.
- Complete assessments like OASIS where permitted, and ensure documentation complies with policies.
- Maintain effective communication with team members and families, serving as a clinical resource.
Qualifications
- Physical Therapy degree from an accredited program.
- Current, unrestricted state license.
- Valid CPR, driver’s license, and proof of auto insurance.
Skills
- Experience in home health therapy and OASIS documentation (preferred).
