About the role
A Physical Therapist (PT) provides patient-centered care in a home health setting, enabling recovery from illness or injury at home.
Responsibilities
- Conducting initial and ongoing assessments of mobility, strength, and function.
- Administering, evaluating, and interpreting tests to set functional goals.
- Creating and updating treatment plans, documenting progress, and modifying goals as needed.
- Delivering prescribed physical therapy services and ensuring effective discharge planning.
- Collaborating with healthcare teams, physicians, and families.
- Supervising other personnel (PTA, HHA) as applicable and completing required assessments.
- Maintaining documentation that complies with policies and regulations.
Qualifications
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license.
- Valid CPR, driver’s license, and proof of auto insurance.
- Preferred: Experience in home health therapy and OASIS documentation.
