Home Health Physical Therapist - Full Time

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and function.
• Develop and implement individualized plans of care with measurable goals and outcomes.
• Deliver prescribed physical therapy interventions and document progress accurately.
• Perform discharge planning and educate patients and caregivers on home exercise programs.
• Collaborate with healthcare teams, including physicians and caregivers, to coordinate services.
• Supervise other personnel such as PT Assistants and Home Health Aides as applicable.
• Complete OASIS assessments where permitted by regulation and ensure compliance with policies and standards.
• Maintain effective communication with staff, patients, and families.

Qualifications & Skills

Degree in Physical Therapy from an accredited program.

Current, unrestricted state license.

Valid CPR certification, driver’s license, and proof of auto insurance.

Preferred: Experience in home health therapy and OASIS documentation.
