About the Role
Seeking a Physical Therapist (PT) to deliver home health care, enabling patients to recover in their own homes.
Responsibilities
- Conduct initial and ongoing patient assessments
- Develop and implement individualized plans of care
- Document patient progress and modify treatment as needed
- Collaborate with healthcare teams to coordinate care
- Educate patients and caregivers on home exercises and safety
- Manage discharge planning
- Supervise staff such as Physical Therapist Assistants (PTAs) and Home Health Aides (HHAs)
- Complete OASIS assessments where permitted
- Adhere to organizational policies and regulatory requirements
Requirements
- Degree in Physical Therapy from an accredited program
- Current state licensure as a Physical Therapist
- CPR certification
- Valid driver’s license
