Home Health Physical Therapist Full Time

Position Overview

A full-time Physical Therapist (PT) is needed to provide home health care services, enabling patients to recover from illness or injury in their own homes.

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and function.
• Develop and implement personalized plans of care with clear goals and outcomes.
• Deliver physical therapy services per physician orders, document progress, and modify treatment plans as needed.
• Cookordinate with healthcare teams, patients, and caregivers, and educate on home exercise programs.
• Participate in discharge planning and supervise other personnel such as PTAs or HHAs where applicable.
• Complete assessments like OASIS where permitted, ensuring compliance with policies, regulations, and documentation standards.

Qualifications And Skills

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license.
• Valid CPR, driver’s license, and proof of auto insurance.
• Preferred: Experience in home health therapy and OASIS documentation.
