About the role
Seeking a Physical Therapist (PT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans.
Responsibilities
- Perform initial and ongoing assessments of mobility, strength, and function.
- Administer tests to set functional goals and provide written evaluations and care plans.
- Deliver therapy services per physician orders.
- Document progress, modify goals, and plan discharges.
- Educate patients and caregivers on home exercises.
- Collaborate with healthcare team members and maintain effective communication.
- Supervise personnel such as PTAs and HHAs.
- Complete necessary assessments like OASIS where permitted.
- Adhere to policies, standards, licensure, and regulations.
Requirements
- Degree in Physical Therapy from an accredited program
- Unrestricted state license in practice location
- Valid CPR certification
- Valid driver’s license and auto insurance

- Experience with OASIS documentation
