Home Health Physical Therapist - Full Time

Position Overview Seeking a Physical Therapist (PT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Responsibilities include performing initial and ongoing assessments of mobility, strength, and function; administering tests to set functional goals; providing written evaluations and care plans; delivering therapy services per physician orders; documenting progress, modifying goals, and planning discharges; and educating patients and caregivers on home exercises. The PT collaborates with healthcare team members, maintains effective communication, supervises personnel such as PTAs and HHAs, and completes necessary assessments like OASIS where permitted. Adherence to policies, standards, licensure, and regulations is essential.
