Home Health Physical Therapist - Full Time

About the Role

A Physical Therapist (PT) provides home-based rehabilitation services to help patients recover from illness or injury in their own homes.

Responsibilities

• Conduct initial and ongoing assessments of patients
• Develop and implement personalized care plans
• Deliver prescribed therapy interventions
• Document patient performance and progress
• Collaborate with healthcare teams to coordinate care
• Supervise relevant personnel (e.g., PT assistants, aides)
• Complete required assessments such as OASIS where permitted

Requirements

• Degree in Physical Therapy from an accredited program
• Current unrestricted state license in Physical Therapy
• CPR certification
• Valid driver’s license and auto insurance

Preferred Qualifications

• Home health experience
• OASIS documentation skills
