Home Health Physical Therapist - Full Time
About the role
We are seeking a Physical Therapist (PT) to assess, teach, and deliver therapy interventions aligned with physician orders within a home health setting. The role involves conducting initial and ongoing assessments of mobility, strength, and function, developing treatment plans, and documenting progress.
Responsibilities
The PT will provide therapy services, modify goals as needed, and collaborate with healthcare team members, patients, and caregivers to ensure optimal outcomes. The position requires supervising other personnel (PTA, HHA), completing assessments where permitted, and maintaining compliance with policies, procedures, and legal standards.

Qualifications
