About the role
SummitWest Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community.
Responsibilities
- Perform comprehensive patient assessments
- Develop and update individualized plans of care
- Provide skilled nursing care and appropriate interventions
- Monitor patient progress and report changes to the care team
- Educate patients and families on medications, treatments, and care plans
- Cookbook: Coordinate care with LPNs, therapists, caregivers, and providers
- Complete, timely, and accurate documentation in EMR
- Participate in on-call rotation
Requirements
- Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license)
- Valid driver's license, reliable transportation, and auto insurance
- Strong assessment and critical thinking skills
- Ability to work independently and manage a structured caseload
- Clear, professional communication with patients and team members
- Comfort with documentation and EMR systems (HCHB preferred)
- Home health experience preferred, but not required
