Home Health Licensed Practical Nurse - Part Time PRN

About the role

Provide skilled nursing care in patients' homes, following care plans and physicians’ orders, while maintaining confidentiality and professional boundaries.

Responsibilities

• Deliver quality patient care under RN supervision, document services, and communicate with physicians and caregivers.
• Recognize patient condition changes, implement interventions to mitigate risks, and educate patients and families based on individual needs.
• Perform various home care skills such as wound care, infusions, catheter care, PICC line dressing changes, and phone triage.

Requirements

• Active LPN license in the applicable state.
• Valid driver’s license, auto insurance, and CPR certification.
• Preferred: Experience with Home Care Home Base platform.
