Home Health Registered Nurse - Part Time PRN

Role: Registered Nurse (Home Health) - Part-Time, PRN, serving Madison and Jackson Counties. Responsibilities Provide individualized, in-home nursing care to promote recovery, safety, and independence.Conduct comprehensive assessments and develop patient-centered care plans.Perform skilled interventions such as wound care, infusions, medication management, and post-operative support.Educate and empower patients and caregivers for symptom, medication, and daily care management.Monitor patient progress, identify early changes, and communicate with the care team to adjust plans.Coordinate care across providers to ensure continuity and high-quality outcomes.Complete accurate documentation to support compliance and measurable results. Qualifications & Skills Active RN license in the practice state.At least one year of clinical nursing experience.Patient-focused, organized, and motivated to deliver high-quality care.Valid driver’s license, reliable transportation, and auto insurance.CPR certification or ability to obtain it.
