Home Health Registered Nurse - Part Time PRN

Key Responsibilities

• Serving as the primary clinical partner for patients and families, delivering personalized nursing care at home to promote recovery, safety, and independence.
• Conducting comprehensive in-home assessments to develop patient-centered care plans.
• Providing skilled interventions such as wound care, infusions, medication management, and post-operative care tailored to individual needs.
• Educating patients and caregivers to manage symptoms, medications, and daily care confidently.
• Monitoring patient progress, identifying early changes, and communicating with the care team to adjust plans and prevent hospitalizations.
• Ensuring care coordination and completing accurate documentation to support quality and compliance.

Qualifications And Skills

• Active RN license in the practice state.
• At least one year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Reliable transportation, valid driver’s license, and CPR certification (or ability to obtain).
