Responsibilities

• Deliver quality patient care under the supervision of a Registered Nurse, including documentation and communication with healthcare providers.
• Recognize and respond to changes in patient needs to prevent or reduce risks.
• Educate patients and families based on individual needs.
• Perform in-home care tasks such as phone triage, wound management, infusions, catheter care, and PICC line dressing changes.

Qualifications & Skills

• Active LPN license in the applicable state.
• Valid CPR, driver’s license, and automobile insurance.
