Role: Licensed Practical Nurse (LPN) - Part Time PRN Responsibilities: Provide and document skilled nursing care per care plans and physicians’ orders, ensuring confidentiality and professionalism. Deliver quality patient care under the supervision of a Registered Nurse, communicate effectively with physicians and caregivers, recognize patient needs and respond to changes, educate clients and families, and perform home care tasks such as wound care, assessment, and phone triage. Qualifications & Skills Active LPN license in the applicable stateValid CPR certificationPreferred: Medicare Skilled Nursing experience, basic Oasis knowledge, 1-year LPN experience in home health or healthcare, Home Care Home Base familiarity Key Duties: Ensure compliant, personalized patient care, monitor patient responses, and implement interventions as needed.
