About the Role
As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes. You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.
Responsibilities
- Provide direct patient care in accordance with the state Nurse Practice Act
- Implement and support the plan of care developed by the RN
- Deliver skilled nursing services, including treatments, medication support, and wound care
- Monitor patient condition and report changes to the RN and care team
- Educate patients and caregivers on self-care, treatments, and disease management
- Maintain infection control and proper use of equipment and supplies
- Document patient care accurately and timely within the EMR
- Communicate effectively with physicians, nurses, and interdisciplinary team members
- Participate in case conferences, team meetings, and ongoing training
- Support coordinated care by maintaining consistent communication across the care team
Requirements
- Active LPN/LVN license in the state of practice
- Graduate of an accredited practical/vocational nursing program
- Current CPR certification
- Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus)
- Strong clinical and observational skills
- Clear communication and patient education abilities
- Ability to follow care plans while recognizing and reporting changes in condition
- Basic proficiency with EMR systems
- Valid driver’s license and reliable transportation
Skills
- Experience in home health or community-based care
- Strong patient engagement and relationship-building skills
- Ability to work independently while staying connected to a team
- Familiarity with Homecare Homebase (HCHB)
