Home Health Licensed Practical Nurse LPN Part Time PRN

About the role

The Licensed Practical Nurse (LPN) Part Time PRN provides skilled nursing care in patients' homes, following care plans and physicians' orders. The role involves hands-on patient care such as wound care, infusions, catheter care, and assessments, under the supervision of a registered nurse.

Responsibilities

• Deliver compliant patient care and communicate effectively with healthcare team
• Monitor patient responses, identify risks, and implement preventive measures
• Educate patients and families based on individual needs
• Perform various home health skills including assessments, wound care, and infusions
• Provide comprehensive home care, document care activities, recognize and respond to patient needs, and educate patients and families to promote health and safety

Qualifications

• Active LPN license in the applicable state
• Valid CPR certification

Skills

• Medicare skilled nursing experience (preferred)
• Basic Oasis understanding (preferred)
• 1+ year home health or healthcare experience (preferred)
• Familiarity with Home Care Home Base (preferred)
